29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about December 13, 2019, employees performing masonry activities from a 3 tier tubular framed scaffold system were not provided a hook on ladder or safe means of access and egress. Employees used the sides and cross braces exposing them to fall hazards up to 18 feet to the ground below.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about December 13, 2019 and at times prior thereto, employees performing masonry activities from scaffold systems that were not fully guarded on the ends and open sides, were exposed to unprotected fall hazards up to 20 feet to the ground below.
