29 CFR 1910.1030(d)(2)(viii): Immediately or as soon as possible after use, contaminated reusable sharps were not placed in appropriate containers until properly reprocessed: Treatment Area: On or about January 10, 2013 and at times prior therto, employees indicated that used reuseable sharps would be hand carried to the dirty side of the set-up area on an as needed basis. This condition exposed employees to sharps injuries from contaminated instruments and exposure to bloodborne-related diseases.

29 CFR 1910.1030(d)(4)(iv)(A)(2): Contaminated laundry was not placed and/or transported in bags or containers which were labeled or color-coded in accordance with 29 CFR 1910.1030(g)(1)(i): Laundry Hamper, Outer Scrubs: On or about January 10, 2013 and at times prior thereto, contaminated outerwear and linens from dental procedures were not placed in containers that were appropriately labeled or color-coded. This condition exposed employees to bloodborne pathogen hazards including hepatitis B and HIV/AIDS.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: Instrument Clean-up Area: On or about January 10, 2013 and at times prior thereto, an employee performing clean-up of dental instruments using Sani Treet Plus disinfectant did not wear appropriate eye protection. This condition exposed the employee to minor eye irritation hazards.
