29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:     Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    On or before January 08, 2013, during construction activities, the employer allowed employees to work in an aerial lift and did not ensure employees were properly attached to the boom of the lift.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or before January 08, 2013, during construction activities, the employer allowed employees to work on an open floor approximately 20 feet high, without any type of fall protection exposing the employees to falls. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, IDENTITY OF PREVIOUSLY CITED STANDARD 29 CFR .501 (b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314587171, CITATION NUMBER 01, ITEM NUMBER 01, AND WAS AFFIRMED AS A FINAL ORDER ON 08/26/2010, WITH RESPECT TO A WORKPLACE LOCATED AT 205 VINE ST., COLUMBUS, OHIO.
