29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    Metal Technology Shop:  On or about 8 January 2013 and times prior thereto, the Standard-Modern Lathe, Model 19/36x54 (Machine 3) did not have a chuck guard installed.  This condition exposed employees to a caught in and/or struck by hazard.

29 CFR 1910.253(b)(2)(ii):     Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons.    Metal technology Shop:  On or about 8 January 2013 and times prior thereto, a 33 gallon propane tank was located in the back portion of the shop not secured.  This condition exposed employees to a struck by hazard.

29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident.    Metal Technology Shop:  On or about 8 January 2013 and times prior thereto, the 120 volt, Circuit Breaker panel (PNL-LB)not fully labeled with all circuits installed.  This exposed employees to an electrical shock hazard.

29 CFR 1910.303(g)(1)(i)(B):     For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater.    Electrical-Environmental Shop:  On or about 8 January 2013 and times prior thereto, the electrical panel in the testing room was blocked.  This condition exposed employees to smoke inhalation hazards in delays in isolating the system.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.    Avionics Shop:  On or about 8 January 2013 and times prior thereto, the 25 foot blue extension cord (shop # A9MVEXTNA) exhibited strain relief on the electrical plug exposing wires.  This condition exposed employees to a 120 volt electrical shock hazard.
