29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: Bassett Hospital, Boiler Room: On or about 08 January 2013 and times prior thereto, the employer did not ensure that horizontal shafting was guarded. Coolant pump motor shafts were not guarded, exposing employees to rotating parts. This condition exposed employees to a caught in hazard.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: Bassett Hospital, CEP: On or about 08 January 2013 and times prior thereto, the employer did not ensure that used openings in electrical boxes and panels were effectively closed. Open blank was covered with cardboard. This condition exposed employees to an electrical shock hazard.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they were in their work area(s) Bassett Hospital, CEP: On or about 08 January 2013 and times prior thereto, the employer did not ensure that safety data sheets for each hazardous chemical was readily accessible in the work area. Random samples of chemicals were selected [NOCO E404S Battery Cleaner and Acid Detector Aerosol - 14 oz. Great Stuff Foam Sealtant - 16 0z. BOSS 813 Expanding Firestop Foam- 24 oz. and 3M Super 77 Multipurpose Adhesive- 16.75 oz.] ;not all chemicals had safety data sheets available. This condition exposed employees to chemical burns, irritation of the eyes and upper respiratory tract, and skin dermatitis.
