WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 471841, CITATION NO. 1, ITEM NO. 1, AND WAS AFFIRMED AS A FINAL ORDER ON 07/16/12, WITH RESPECT TO A WORKPLACE LOCATED AT 10 SHERIDAN DRIVE, TONAWANDA, NY, 14150. a) On or about 01/14/13, in the Department 227, while performing changeovers and shut downs on the Quadraplex Extruder Machine, employees were exposed to caught between hazards created during the closing phase of this machine's clam/head locks. NO ABATEMENT CERTIFICATION REQUIRED
