29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a)    Lambda Line: On or about January 17, 2013: an employee doing quality control was exposed to continuous noise at a level of 75.5% (equivalent to an 8 hour TWA of 87.9 dBa) which is approximately 1.51 times the Action Level of 50% (equivalent to an 8 hour TWA of 85dBa) Results are based on a 392 minute sampling period conducted. Exposure calculations include a zero increment for the 88 minute period not sampled    b) YFA Line: On or about January 17, 2013: an employee operating the side bracket welder was exposed to continuous noise at a level of 72.6% (equivalent to an 8 hour TWA of 87.6 dBa) which is approximately 1.45  times the Action Level of 50% (equivalent to an 8 hour TWA of 85dBa) Results are based on a 480 minute sampling period conducted.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a)    Lambda Line Spring Seat: On or about January 10, 2013: the employer exposed his employee(s) to potential hazardous energies in that employees were allowed to work on the robotic welding machine(s) which has multiple energy sources such as 220 volt 3 phase electrical, hydraulic energy and compressed air without those machines having a written machine specific lock out procedure to ensure that those energy sources were controlled while employees were performing maintenance or servicing the machine.

The Daeil USA Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 581398 citation number 1, item number 2 and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 3905 45th St. SW, Lanett, AL 36863.

The Daeil USA Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.252(b)(2)(iii), which was contained in OSHA inspection number 316304310 citation number 1, item number 1 and was affirmed as a final order on May 1, 2012, with respect to a workplace located at 3905 45th St. SW, Lanett, AL 36863.
