29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate. LRS Warehouse Area, Center Mezzanine: On or about 9 January 2012 and times prior thereto, the center mezzanine did not have a load rating for the structure. This condition exposed employees to a struck by hazard in the event that materials were stored over the capability of the roof to support it.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater. LRS Warehouse, Outside storage area: On or about 9 January 2013 and times prior thereto, the electrical panel in the outside storage area was blocked. This condition exposed employees to electrical and or smoke inhalation hazards due to delays in isolating the system.
