29 CFR 1910.29(f)(1):     "Design." Units shall comply with applicable requirements of paragraph (a) of this section.    Warehouse Area:  On or about 10 January 2013 and times prior thereto, the Ballymore 6 1/2 feet ladder stand had a bent frame and was missing the rubber stops.  This condition exposed employees to a fall hazard.

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.    Deli / Sushi Bar Area: On or about 10 January 2013 and times prior thereto, a multi plug power strip was hanging by its electrical connection, damaging the wiring.  This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.303(g)(1)(ii):     The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:    Warehouse Electrical Room:  On or about 10 January 2013 and times prior thereto, the Electrical Room near the meat department was used for storage.  This condition exposed employees to smoke inhalation hazards in delays in isolating the system.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).    a)  Bakery:  On or about 10 January 2013 and times prior thereto, the cooler in the bakery had exposed wired on top of the unit.  This condition exposed employees to a 120 volt electrical shock hazard.    b)  Store Electrical Room:  On or about 10 January 2013 and times prior thereto, a 120 volt electrical outlet face was broken.  This condition exposed employees to a 120 volt electrical shock hazard.    c)  Meat Cutting Room:  On or about 10 January 2013 and times prior thereto, a 120 volt electrical outlet cover was missing.  This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.304(b)(3)(i):     All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops shall have ground-fault circuit-interrupter protection for personnel.    Delicatessen (Sink Area):  On or about 10 January 2013 and times prior thereto, a ground-fault circuit-interrupter did not function properly.  This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident.    Store Front Electrical Room:  On or about 10 January 2013 and times prior thereto, electrical circuit breaker panel #CLI was not fully labeled with the identification of all circuits installed.  This exposed employees to an electrical shock hazard.    The Defense Commissary Agency was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307507822, Citation 01, Item Number 03, issued 03 June 2011, with respect to a workplace located at Eielson AFB, Alaska.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).    Produce Processing and Storage Area:  On or about 10 January 2013 and times prior thereto, the 120 volt, Clarke vacuum, serial # 0814074421 was missing the ground pin.  This exposed employees to an electrical shock hazard.    The Defense Commissary Agency was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307505404, Citation 01, Item Number 03a, issued 22 January 2010, with respect to a workplace located at Fort Wainwright CDC, Alaska.
