29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times. Back Warehouse and Freezer Storage corridor: On or about 10 January 2013 and times prior thereto, four of the five emergency exit lighting were inoperable. This exposed employees to delays in existing the building causing smoke inhalation and fire hazards.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. Meat Processing Room: On or about 10 January 2013 and times prior thereto, the Hobart meat cutting machine was still plugged into an electrical source with the blades removed and prepared for cleaning. This condition exposed employees to a caught in hazard.
