29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.      (a)  Backside of Scaffold - On or about January 3, 2013 an employee was exposed to falling objects while other employees were working directly above him handling cinder blocks and mortar.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    (a)  Backside of Building - On or about January 3, 2013 employees were exposed to a fall hazard while climbing on a scaffold that did not any form of access or egress. No ladder was installed to get on or off the scaffold. Employees climbed the frame of the scaffold. The scaffold was 12 feet 6 inches above the ground.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.      (a)  Backside of Building - On or about January 3, 2013 one employee was exposed to a fall hazard while standing on a scaffold 12 feet 6 inches above ground. Employee was not protected from falling to a lower level by a guardrail system or personal fall arrest system.       (b)  North Concrete Masonry Unit Wall - On or about January 3, 2013 one employee was elevated on a wooden pallet attached to the forks of Caterpillar TL642 Telehandler Forklift (serial JR4552) 16 feet above ground. Employee was not protected from falling to a lower level by a guardrail system or personal fall arrest system.
