29 CFR 1910.29(f)(1):     "Design." Units shall comply with applicable requirements of paragraph (a) of this section.      Warehouse Area:  On or about 9 January 2013 and times prior thereto, the Yellow 5-step portable ladder stand was missing the rubber floor cushions.  This condition exposed employees to a fall hazard.

29 CFR 1910.101(b):     The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    Warehouse area:  On or about 9 January 2013 and times prior thereto, a gaseous Helium cylinder was not secured.  This condition exposed employees to a struck by hazard.

29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident.    Labeling Room, Warehouse area:  On or about 9 January 2013 and times prior thereto, the electrical panel was not labeled identifying the circuits and what they controlled.  This exposed employees to an electrical shock hazard.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).    a) Warehouse area:  On or about 9 January 2013 and times prior thereto, the electrical lighting above the empty box storage area and exposed wires.  This condition exposed employees to 120 volt electrical shock and burn hazards.    b) Warehouse area. Center wall area:  On or about 9 January 2013 and times prior thereto, an electrical outlet and junction box had exposed wires.  This condition exposed employees to 120 volt electrical shock and burn hazards.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws      Warehouse area:  On or about 9 January 2013 and times prior thereto, an orange extension cord exhibited damaged strain relief on the electrical plug, exposing wires.  This condition exposed employees to a 120 volt electrical shock hazard.
