29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm    Warehouse:  On or about 9 January 2013 and times prior thereto, the employer did not have a fall prevention program nor provided training on the use of fall protection equipment for employees who operated the JLG 20MSP.  This condition exposed employees to a fall hazard.    Among other methods, a means of abatement for this condition would be to develop a fall protection program for AAFES employees who operate this equipment; and ensure they are trained on the correct use of the fall arrest equipment.

29 CFR 1910.22(d)(1):     On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.    AAFES Warehouse:  On or about 9 January 2013 and times prior thereto, the mezzanine above office spaces adjacent to the second floor did not have a load rating.  This condition exposed employees to a stuck by hazard.      AAFES was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307502997, Citation 02, Item Number 01, issued 30 May 2008, with respect to a workplace located at Eielson AFB, Alaska.
