29 CFR 1910.22(d)(1):  On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed to the building:      a)  On or about January 15, 2013, and at times prior, employees were exposed to struck-by or crushed-by hazards while they worked below materials that were stored on the ceiling of the main office/breakroom area and several other storage rooms.  The employer had not posted the load rating for the roof/ceiling structure.

In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy:    a)  On or about January 15, 2013, and at times prior, employees were exposed to electrical shock while they worked around a junction box next to a back door of the main warehouse/office building which did not have an adequately installed cover.

In particular, the employer did not have an MSDS for the corrosive Red Thunder used to wash vehicles.

29 CFR 1904.32(a)(1): The employer did not review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified:    a) The employer did not ensure that a company officer reviewed the OSHA 300 Log for 2010 and signed the 300A annual summary form certifying that entries were complete and accurate.  One injury on the log was reported as an "other recordable" case but showed 26 days of job transfer or restriction.    b)  The employer did not ensure that a company officer reviewed the OSHA 300 Log for 2010 and signed the 300A annual summary form certifying that entries were complete and accurate.  The 300A summary form reported that employees had worked 129,684 total hours during the year when the actual hours worked was 60,448.
