29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) On or about January 14, 2013, machine guarding did not protect the operator from the point-of-operation posed by the rotating sealer bar on the Conflex Horizontal Wrapper (Serial #CO603-208). The magnetic interlock switch was inoperative.
