29 CFR 1910.119(f)(1)(i)(E): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information that addressed emergency operations. (a) Location - Polyurethane Department: On or About 1/15/2013, work instruction, "PU-018 - Discharge of SFA From Reactor to Appropriate Container," did not provide clear instructions related to emergency operations. The instruction section on emergency operations said N/A. Bostik, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.119(f)(1)(i)(E), which was contained in OSHA inspection number 315298307, citation number 1, item number 9b and was affirmed as a final order on May 14, 2012, with respect to a workplace located at 211 Boston St., Middleton, MA.
