29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared:    (a) On or about January 14, 2013 - Extrusion Line Operator 2, an employee was exposed to continuous noise levels at 87% of the allowable 8-hour time-weighted average level (85 dBA). The equivalent dBA of the employee is approximately 89 dBA. The sampling was performed for 476 minutes during one shift on January 23, 2013. Zero exposure was assumed for the unsampled time period of 4 minutes.    (b) On or about January 14, 2013 - Extrusion Line Operator 1, an employee was exposed to continuous noise levels at 191% of the allowable 8-hour time-weighted average level (85 dBA). The equivalent dBA of the employee is approximately 94 dBA. The sampling was performed for 480 minutes during one shift on January 23, 2013.     (c) On or about January 14, 2013 - Extrusion Line Operator 3, an employee was exposed to continuous noise levels at 93% of the allowable 8-hour time-weighted average level (85 dBA). The equivalent dBA of the employee is approximately 89.4 dBA. The sampling was performed for 475 minutes during one shift on January 23, 2013. Zero exposure was assumed for the unsampled time period of 5 minutes.     (d) On or about January 14, 2013 - Floater, an employee was exposed to continuous noise levels at 105% of the allowable 8-hour time-weighted average level (85 dBA). The equivalent dBA of the employee is approximately 90.3 dBA. The sampling was performed for 466 minutes during one shift on January 23, 2013. Zero exposure was assumed for the unsampled time period of 14 minutes.
