29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about 9/18/2019 - Extrusion Department at Line 3 Slitter, employees were exposed to caught-in hazards due to a guard missing at the ingoing nip point created by the web and the primary roller.

29 CFR 1910.145(f)(5): Danger tags were not used where an immediate hazard presented a threat of death or serious injury to employees: (a) On or about 9/18/2019 - Extrusion Department, Lines 1, 2, 3, and 5, danger signs were not posted at the new plexi-glass guards that had been installed at the Godet roll stands to warn employees of caught-in hazards and that the guards must be in place before operating the machines.
