The employer removed the lower outflow door which is designed to be mounted on the Rietz Blender. The black colored danger sign was affixed to the outflow door. b. The employer did not instruct all employees as to special precautions necessary when working in the vicinity of the Rietz Blender.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 01/14/13 at Dawsonville, GA: Employees were exposed to machine related hazards, while working in the Ground Department. The employer failed to provide proper machine guarding in the following instances: b) Ground Department (Rietz Blender): Employees were exposed rotating parts within the Rietz blender. The employer failed to properly guard the Rietz blender's lower left discharge door during operation. c) Ground Department (Back Dump): Employees were exposed to struck-by hazards while performing work in the vicinity of the Back Dump operation. The employer failed to guard the area beneath the Back Dump, when the back dump rotated from the vertical to horizontal position.

The length of an unguarded rotating shaft located at the end of a conveyor system, adjacent to the Rietz Blender, was less than one half of its diameter.
