29 CFR 1915.56(b)(4): When welding cable became worn to the extent of exposing bare conductors, the portion exposed was not protected by means of rubber tape, friction tape, or other equivalent insulation: FSV-2: On or about January 8, 2013 and times prior thereto, a welding cable running along the starboard side of the vessel had two cuts to the conductor. This exposed employees to burn/shock hazards if they were to contact the conductors.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: FSV-2: On or about January 8, 2013 and times prior thereto, the temporary lighting electrical cord was not protected from damage from the vessel door. This exposed employees to possible shock if the cord were to get damaged from the door closing on it.

This exposed employees to potential shock hazards from incidental contact with the conductor.
