29 CFR 1915.89(c)(2): The employer did not ensure the use of a lock to prevent energizing or startup, or the release of hazardous energy, before any servicing is started, unless the employer can demonstrate that the utilization of a tags-plus system will provide full employee protection as set forth in paragraph (c)(6) of this section. FSV-7 Engine space: On or about January 8, 2013, and times prior thereto, the #1 and #2 sanitary flush pumps only had tags installed with no other protection. This exposed employees to a potential electrical shock hazard from the system not providing full employee protection.

29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: FSV-7: On or about January 8, 2013, and times prior thereto, an employee did not have a current fit-test. This exposed the employee to possible inhalation hazard from a tight fitting respirator not fitting correctly.
