29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace. FSV-2 engine space: On or about January 8, 2013 and times prior thereto, employees wore a tight fitting respirator and had not had a medical evaluation. This exposed them to increased health risks from wearing a respirator.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test. FSV-2 engine space: On or about January 8, 2013 and times prior thereto, employees had not been fit tested for using a respirator when the employer required there use. This exposed employees to potential inhalation hazards from not having a respirator with a proper fit.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve. FSV-2 engine space: On or about January 8, 2012 and times prior thereto, a full face respirator not in use was not stored properly. This exposed employees to inhalation hazards from wearing a respirator with contamination.
