29 CFR 1926.652(c)(2)(ii): Deviation from the specifications, recommendations, and limitations issued or made by the manufacturer were allowed without written approval from the manufacturer: A. Peterson Construction Company worksite located near the intersection of Fremont Pike & Crossroads Parkway, Rossford, Ohio: On or about January 18, 2013, the employer failed to attain specific written approval from the manufacture of the Kundel Trench Shield for using steel plates as end protection. The steel plates were placed against the spreader bars. Employees were exposed to potential crushed-by hazards due to the potential of added pressure on the shield by exceeding the tabulated data of the manufacturer.
