29 CFR 1910.178(l)(2)(ii):  Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    Toddco Plastics, Inc. had employees operating forklifts daily at the facility, and the employer did not provide forklift training that included the following:    a.	Practical training (demonstrations performed by the trainer and practical exercises performed by the trainee) and  b.	Evaluation of the operators performance in the workplace.

29 CFR 1910.178(q)(7):  Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.    Toddco Plastics, Inc. had employees operate powered industrial trucks daily, and the employer did not conduct daily forklift inspections.

29 CFR 1910.305(b)(2)(i):  All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    Toddco Plastics, Inc. had an employee exposed to electrical hazards from a missing electrical cover for an electrical outlet in the companys office.

29 CFR 1910.147(c)(4)(i):  Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     Toddco Plastics, Inc. had employees perform maintenance operations within the facility, and the employer did not develop, document, and utilize energy control procedures.  These energy control procedures would include the following:    a.	A specific statement of the intended use of the procedure,  b.	Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy,   c.	Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and  d.	Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    Toddco Plastics, Inc. had employees perform maintenance operations within the facility, and the employer did not conduct an energy control inspection to ensure that all hazardous energy was controlled during maintenance operations and the requirements of 1910.147 were met.

29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.      Toddco Plastics, Inc. had employees perform maintenance operations within the facility, and the employer did not provide lockout training to ensure all employees had the knowledge and skills to control all hazardous energy during maintenance operations.  This training would include training in the following:    a.	Authorized employees  recognition of hazardous energy sources, type and magnitude of energy available, and methods and means necessary for energy isolation and control,  b.	Affected employees  purpose and use of the energy control procedures, and   c.	Other employees  instructed about procedure and prohibition relating to attempts to restart or reenergize machines or equipment.

29 CFR 1910.147(f)(3)(ii)(D):  Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.    Toddco Plastics, Inc. had two employees changing a hydraulic pump on Van Dorn 1100 injection mold press in December of 2012, and only one of the employees applied a lock to the 480 electrical disconnect.
