29 CFR 1910.305(a)(2)(v)(A): Branch circuits did not originate in an approved power outlet or panelboard: Instance 1: On or about 01/22/2013, in the Store Managers office, the employer did not ensure that the power strip and multi-outlet extension cord utilized to provide electricity to several pieces of office equipment originated in approved power outlets; used an extension cord in a mixed daisy-chain configuration to provide electricity to a six (6) outlet power strip and four (4) outlet extension cord connected in series. Instance 2: On or about 01/22/2013, in the Guard Gate office, the employer did not ensure that the multi-outlet extension cord utilized to provide electricity to office equipment originated in an approved power outlet; used a three (3) outlet extension cord connected in series (daisy-chained) to a six (6) outlet power strip.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above an adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there was entrance to a ramp, stairway, or fixed ladder: On or about 01/22/2013, in the Insulation Area of the Yard, the employer failed to ensure that employees who retrieved water heaters were protected from falling approximately twelve (12) feet to the ground level; a chain was installed along the open side. STINE, L.L.C., DBA STINE LUMBER WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, 29 CFR 1910.23(c)(1), WHICH WAS CONTAINED IN THE OSHA INSPECTION NUMBER 541918, CITATION NUMBER 01, ITEM 001, ISSUED 09/25/2012, WITH RESPECT TO THE WORKPLACE LOCATED AT 4501 NELSON ROAD, LAKE CHARLES, LA 70605. THIS CASE WAS AFFIRMED AS A FINAL ORDER ON 10/30/2012.

Instance 2: On or about 01/23/2013, in the Guard Gate office, the employer failed to ensure that a six (6) outlet power strip utilized to provide power to office equipment (computer, fan, chargers) was installed and used according to its listing; the power strip was mounted to wood paneling under the desk. Instance 3: On or about 01/23/2013, in the Store Manager's office, the employer failed to ensure that an eight (8) outlet power strip utilized to provide power to office equipment was installed and used according to its listing; the power strip was mounted to the wall.
