29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) American Turned Products, Inc. / 7626 Klier Drive in Fairview, PA: On or about January 23, 2013, specific lockout tagout procedures were not developed, documented, and implemented for the machinery and equipment. The lockout/tagout procedures did not identify specific information noted in the 29 CFR 1910.147(d). In addition, the lockout/tagout procedures lacked information that included but was not limited to: (1) Specific methods for verifying isolation to ensure the machine is completely de-energized (2) Energy sources and their magnitudes (3) Energy isolation points (4) Methods to dissipate stored/residual energy (5) Restart procedures. The procedures that were lacking specific information include, but are not limited to, Haas Vertical Mill, Durr 71W, Durr 81C, Novi Staker, Hydromat and Debur machine.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; or a) American Turned Products, Inc. / 2626 Klier Drive, Fairview, PA 16415: On or about January 23, 2013 the maintenance shop area had an oxygen and fuel cylinder stored next to each other.
