OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards when using an overhead hoist to lift loads without a latch on the hook: a) Murotech Ohio Corporation, located at 550 McKinley Rd., Saint Marys, OH 45885: On or about January 23, 2013, the employer did not ensure that the 2-ton Coffing hoist (Serial # EC3E180WMD), located on the West side of the press room, was outfitted with a hook equipped with a safety latch, exposing employees to struck-by hazards in the case when the lifting device could be dislodged from the hook. No additional abatement information is needed for this item.

A. Murotech Ohio Corporation, located at 550 McKinley Rd., Saint Marys, OH 45885: On or about January 23, 2013, the employer failed to ensure that the pendant control, used with the 2-ton Coffing hoist (Serial # EC3E180WMD), located on the West side of the press room, was properly maintained. The employer did not ensure that all of the pendant control buttons were properly labeled, exposing employees to struck-by hazards in the case when the crane could be moved in the unintended direction. B. Murotech Ohio Corporation, located at 550 McKinley Rd., Saint Marys, OH 45885: On or about January 23, 2013, the employer failed to ensure that the pendant control, used with the 2-ton Coffing hoist (Serial # EC3E152YED), located on the East side of the press room, was properly maintained. The employer did not ensure that all of the pendant control buttons were properly labeled, exposing employees to struck-by hazards in the case when the crane could be moved in the unintended direction. No additional abatement information is needed for this item.

Additional inspections were not performed during sling use, where service conditions warrant. Damaged or defective slings were not immediately removed from service. a) Murotech Ohio Corporation, located at 550 McKinley Rd., Saint Marys, OH 45885: On or about January 23, 2013, the employer failed to ensure that the wire rope quadruple sling, used on Line 300 in the press room, was inspected each day before being used for damage or defects by a competent person designated by the employer. Employees were exposed to struck-by hazards in the case if the sling was damaged or defective and not capable of handling the rated load capacity. No additional abatement information is needed for this item.

Employees were exposed to struck-by hazards in the case if the sling was loaded above its rated capacity. No additional abatement information is needed for this item.
