St. Marys, OH 45885; On or about February 15, 2013, the employer did not assure the CAT Forklift Model C5000 only handled loads within the rated 4750 lb capacity. Employees were exposed to forklift related hazards while transporting two 1200kg (or approximately 5291lb) steel coils on a pallet to and from the steel coil storage area.

St. Marys, OH 45885; On or about February 15, 2013, the employer did not assure press dies were stamped to indicate weights. Employees were exposed to potential overloading of forklifts while moving the dies to and from storage.
