29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about January 14, 2013, employees were potentially exposed to serious injury from a pipe threader that was not equipped with a guarded foot switch, or other momentary contact device. The pipe threader was located in the NW corner of building 72 rigging loft, at 5555 N Channel Ave in Portland, OR.

The drill press was located in the SW corner of the NTS bay, of building 72, at 5555 N Channel Ave in Portland, Oregon.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. Bay 10, Tool Room - (a) The tongue guard on the Jet 3/4 h.p. bench grinder was improperly adjusted, exposing employees to flying abrasive wheel fragments in the event of the wheel breaking up.
