29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. a. South employee entrance/exit - On or about January 24, 2013, the south employee exit door has a lock that requires special knowledge to operate, thereby restricting passage in the event of an emergency.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a. Compounding department - On or about January 24, 2013, the north exit in the compounding department was obstructed due to pipes lying across the exit route in front of the exit door. Employees were exposed to obstructed exits from stairs and/or a ramp not being provided when the exit route was not level.

29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a. On or about January 24, 2013, in the facility, employees were required to work with heated water, cleaning solutions and product. The employer failed to provide adequate body protection including personal protective equipment which protected the full upper torso and lower legs against contact with the heated materials.
