29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: On or about August 9, 2014, at the Compounding Department, a temporary employee required to clean floors with hot water suffered a second degree thermal burn to the right thigh. The employer did not provide and/or require the employee to wear appropriate protective equipment, including protective clothing for the torso and legs, to protect against thermal burn injury. Vee Pak, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 844723, citation number 1, item number 2, and was affirmed as a final order on March 13, 2013, with respect to a workplace located at 5331 S. Dansher Road, Countryside, IL. 60525.

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report: On or about August 9, 2014, at the Compounding Department, a temporary employee required to clean floors with hot water suffered a second degree burn to the right thigh. The employer did not record the injury on the OSHA 300 and 301 forms, or equivalent forms.
