Employees working from a 5 in 12 pitch roof were exposed to falls of approximately 9 feet 2 inches to a lower level. The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Employees working from a 5 in 12 pitch roof were exposed to falls of approximately 9 feet 2 inches to a lower level. Employees were not protected from falling by guardrail systems, safety net systems, or a personal fall arrest systems or any other approved means. Barringer Brothers Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313016032, citation number 2, item number 1 and was affirmed as a final order on 2 November 2010, with respect to a workplace located at 917 Pacific Crossing Dr. O Fallon, IL 62269.
