29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: (a). On or about January 18, 2013, employees installing plywood roof decking on a 6:12 pitch roof of a one-story single family home being constructed at Lot 106 of the D.R. Horton development in Apopka, FL, were not using conventional or alternative fall protection exposing them to a 12-foot fall hazard. (b). On or about January 29, 2013, employees installing plywood roof decking on a 5:12 pitch roof of a two-story single family home being constructed at Lot 109 of the D.R. Horton development in Apopka, FL, were not using conventional or alternative fall protection exposing them o a 22-foot fall hazard. (c). On or about March 1, 2013, employees installing plywood roof decking on a single story 5:12 pitch roof of a single family home located at 1106 Legg Drive of the D.R. Horton development in Apopka, FL were not using conventional or alternative fall protection to protect them from a 10-foot fall hazard.
