29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a)  An empoyees installing roof sheeting onto a 5:12 pitched roof were not protected from a fall hazard of 10 feet by the use of conventional or alternative fall protection as observed on or about April 22, 2013.

29 CFR 1926.20(b)(2):  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a).  The employer had not conducted frequent inspections of his job site, equipment and materials being used by employees required to perform roofing construction which exposed them to hazards such as but not limited to falls and electrical shock.

29 CFR 1926.404(b)(1)(iii)(C):  Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  (a). The power cord used to provide 110 Volts AC to the hand held skill saw being used by an employee installing plywood sheeting onto the roof being constructed on a one story single-family home had visible damage where the outer sleave was damaged exposing the core conductor cables, as observed on or about April 22, 2013.
