29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:    On or about January 24, 2012, employees performing masonry activities on a residential home were working on a tubular scaffold system and the scaffold legs were erected on stone blocks, making the scaffold system unstable and exposing employees to the hazard of falling approximately 14 feet from the ground.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.    On or about January 24, 2013, employees performing masonry activities on a residential home accessed the scaffold system upper work platforms by means of the side rails and cross braces.  The employer did not provide a ladder that was connected to the scaffold system and at least 3 ft above the upper landing surface exposing employees to a fall hazard.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    On or about January 24, 2013, employees performing masonry activities on a residential home were working from the upper platform of a tubular welded scaffold system that did not have guard rails, mid rails or side rails installed on the upper working platform.  Employees were exposed to a fall hazard approximately 14 feet to the ground and stone below.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toe boards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    On or about January 24, 2013, employees performing masonry activities on a residential home from a tubular welded scaffold system did not have toe boards installed to protect employees from head injuries from falling stones, brick or tools.
