29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) Jobsite: On or about July 12, 2016, two employees were working from a pump jack scaffold and they were not protected from falling to the ground. The fall hazard to the two employees was about 15 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill: (a) Jobsite: On or about July 12, 2016, the two employees working form the pump jack scaffold need to be retrained in how to recognize and minimize fall hazards.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: (a) Jobsite: On or about July 12, 2016, a 28 foot aluminum extension ladder was being used as a staging pic for materials from the pump jack scaffold. In addition, this ladder was being used as a top rail for fall protection for the employees from the pump jack scaffold.
