29 CFR 1910.212(a)(1):  One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a) Bagging Station on the Clover Machine, Suscon Facility, 600 Railway Street, Williamsport, PA: An employee operating a Dyco Model 2020 Semi Auto Bottle Bagger without the use of guards to prevent employee access to the pinch point between the top sealer jaw and the upper support bar was exposed to a caught between hazard on January 7, 2013.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.305(b)(1)(i):  Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed.  a) 6A Mixing Machine, West Wall of Mixing Room, Suscon Facility, 600 Railway Street, Williamsport, PA: The employer did not ensure that the conductors for the flexible power cord entering the Plastic Process Equipment, Inc. Model HL-1 control box on the 6A Mixing Machine were protected from abrasion and that the opening through which the flexible power cord conductors entered the control box was effectively closed exposing employees to an electric shock hazard, on or about January 25, 2013.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.305(g)(2)(iii):  Flexible cords were not connected to devices and fittings so that strain relief was provided that prevented pull from being directly transmitted to joints or terminal screws.  a) 6A Mixing Machine, West Wall of Mixing Room, Suscon Facility, 600 Railway Street, Williamsport, PA: The employer did not ensure that the flexible power cord for the Plastic Process Equipment, Inc. Model HL-1 control box on the 6A Mixing Machine had appropriate strain relief. The flexible power cord was not connected with strain relief and the insulation of the flexible power cord was damaged uncovering the conductors and exposing employees to an electric shock hazard, on or about January, 25, 2013.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
