29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On January 17, 2013, at the jobsite located at 200 Main Street in Smithville MO, employees were exposed to a fall hazard in that they were working on a steep roof approximately 20 feet above ground level without the use of an approved form of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder���s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder: On January 17, 2013, at the jobsite located at 200 Main Street in Smithville MO, employees were exposed to a fall hazard in that the ladder being used to access and egress the roof was not extended three feet above the roof eve, nor was the ladder secured at the top.
