Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On September 18, 2013, at the jobsite located at or near 4610 Antioch Street, in Kansas City, MO, employees were exposed to a fall hazard in that they were working on a roof, approximately 8 feet above next lower level without an approved form of fall protection.
