Section (5)(a)(1) of Public Law 91-596, the Occupational Safety & Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that:  employees were exposed to the hazard of being struck by a new metal sign being suspended above them by a sling from the forks of a Terex Telehandler Lift, whose forks had holes cut/bored through the forks, thereby weakening their lifting capacity.    (a) Word of Life Mission Outreach Center, 1133 West Capitol Street, Jackson, MS 39203 - On or about 1/25/2013 at 1:55 PM employees were exposed to a 500 pound metal sign falling, as it was being installed at the above address, because holes had been cut/bored through the ends of both forks attached to the Terex TX 55-19 Compact Telehandler lift to allow two 2 inch diameter trailer-hitch balls to be mounted to keep the fabric sling from slipping off the weakened fork ends.
