29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about January 9, 2013: (a) An employee was exposed to fall hazards of approximately 20 feet, while installing roofing felt material on a roof with a pitch of 6 in 12. The employee was wearing a rock climber style harness where the attachment point was in the front of the harness.
