29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Lot # 145, 14 Madrid Avenue, Bullsboro Crossing, Newnan, GA- An employee was using an extension ladder to access the roof of a new single family house under construction that was not extended three (3) feet above the landing or tied off to prevent displacement, exposing the employee to fall hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) Lot # 145, 14 Madrid Avenue, Bullsboro Crossing, Newnan, GA- An employee installing shingles on a 6/12 pitched roof with an eave height of approximately 19 feet was not provided with a fall protection system. Sergio Espinoza was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(11) which was contained in OSHA Inspection Number 851183, Citation 1, Item 1a, issued on 03/18/2013 with respect to a work place located at 4670 Fourth Rail Ln, Cummings, GA 30028 and became a final order on 05/20/2013.
