29 CFR 1910.1000(a)(2):  Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million:    a)  St. Elizabeth Medical Center, on or about 1/23/13, an employee plowing snow with a hospital owned Dodge Ram Truck was exposed at a level of 79 parts per million (ppm) of carbon monoxide, over an eight hour time weighted average (TWA), approximately 1.58 times the OSHA permissible exposure limit of 50ppm. The sampling period was for 288 minutes. A zero increment is included for the 192 minutes not sampled. Employee was not wearing any type of  respirator protection.

29 CFR 1910.1000(e):  Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):    a)  St. Elizabeth Medical Center, on or about 1/23/13: Feasible administrative or engineering controls were not implemented to reduce employee exposure to carbon monoxide below the OSHA Permissible Exposure Limit.  Employee exposure as referenced in Citation 1 Item 1a:    EFFECTIVE MEANS OF ABATEMENT INCLUDE BUT ARE NOT LIMITED TO:    1. Improve Preventative Maintenance Program for all hospital owned trucks.     2. Take random carbon monoxide readings with a CO direct reading device to ensure employees are not being overexposed while using hospital owned equipment
