29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about January 28, 2013, north side of the construction site, two extension cords with missing ground pins were utilized to provide power to two Dewalt skill saws. The employees were exposed to electrical hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about January 17, 2013, north side of the construction site, employees exposed to a fall hazard of approximately 10 and 23 feet while engaged in framing activities on a residential home.
