Condition exposed employees to a trip and/or fall hazard of approximately 16 inches.   (b) Cypress Investments, LLC, 2426 Lafayette Street, Denver, CO:  On and prior to January 23, 2013, the controlling employer in charge of the worksite had not ensured appropriate covers were installed over floor holes in the dining room on the first floor.   Condition exposed employees to a trip and/or fall hazard of approximately 16 inches.  (c )  Cypress Investments, LLC, 2426 Lafayette Street, Denver, CO:  On and prior to January 23rd,  2013, the controlling employer in charge of the worksite had not ensured appropriate covers were installed over the stair cutout in the back kitchen area measuring 30 inches X 12 feet long.  Condition exposed employees to a fall hazard of approximatey 7 feet - 3 inches.
