29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms: On or about January 17, 2013, employees working in load out area #3 were exposed to possible fall hazards due to an open-sided stairway lacking a complete stair rail system. The stairway was used to access an upper platform in the tank area and lacked a midrail. Abatement completed during inspection. No abatement documentation required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about January 17, 2013, employees working in load out area #3 were exposed to the hazard of becoming entangled in a rotating part in that a high speed shaft with an irregular (none-smooth) surface held in place with a large hex-head rotating bolt was unguarded and could easily be contacted by workers in the area. Corrected during inspection. Abatement documentation for this item is not required.
