29 CFR 1926.404(f)(7)(iv)(C)(2):     Exposed noncurrent-carrying metal parts of cord- and plug-connected equipment which may become energized were not grounded when used in damp or wet locations or by employees standing on the ground or on metal floors or working inside of metal tanks or boilers:    a) On or about 1/30/13, in the Main Office Area at the Buffalo Zoo New Entry Complex under construction, 300 Parkside Ave., Buffalo, NY: An employee used a non double insulated Arvin Space Heater that had a metal frame, and an "old style, 2-prong plug". The heater was connected to 120-volt ac power via a series of extension cords, the first of which was plugged into a temporary receptacle inside the building.    b) On or about 1/30/13, in the Main Office Area at the Buffalo Zoo New Entry Complex under construction, 300 Parkside Ave., Buffalo, NY: An employee had used a non double insulated portable Patton fan with a metal frame. The fan had been connected to 120-volt ac power via a series of extension cords, including an adapter. The ground prong on the fans' plug was broken off & stuck inside the 3-way adapter.    c) On or about 1/30/13, in the Main Office Area at the Buffalo Zoo New Entry Complex under construction, 300 Parkside Ave., Buffalo, NY: An employee brought a non double insulated, metal framed box fan with a 2-prong plug into the work area.    NO ABATEMENT CERTIFICATION REQUIRED
