On or about December 18, 2012, at the jobsite located at 15500 7th Avenue, Phoenix, IL. - employees performing roofing activities were using a two story staircase that was not equipped with handrails, thereby exposing employees to falls hazards.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a.) On or about December 18, 2012, at the jobsite located at 15500 7th Avenue, Phoenix, IL. - employees performing roofing activities were exposed to falls of approximately sixteen (16) feet seven (7) inches, created by the presence of unprotected sides. JL Adler Roofing and Sheet Metal, Inc. was previously cited for a violation for this Occupational Safety and Health Standard or its equivalent Standard, 29 CFR 1926.501(b)(10), which was contained in OSHA Inspection Number 311374912, Citation Number 1 Item Number 2 and was affirmed as final order on August 19, 2008 with respect to the workplace located at Lincoln Way East High School, Frankfort, IL 60423.
