29 CFR 1926.301(c): Impact tools were not free of mushroomed heads: On or about January 31, 2013 the following tools had mushroomed heads located at the Waller Creek Tunnel Project Inlet Facility at Waterloo Park, found in Austin, Texas: A job made drift pin made from concrete rebar. The drift is used to drive out boom pins. This tool was located in the Kobelco Model 7150 crane.

29 CFR 1926.1433(d)(7)(iii)(C): Windows were not kept free of visible distortion or otherwise obscure visibility that interferes with the safe operation of the equipment. On or about January 31, 2013 the top window in the cab of a Link-Belt model LS-338 crawler crane was cracked and dirty. The crane was located at the Waller Creek Tunnel Project Inlet Facility at Waterloo Park, found in Austin, Texas.
