29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. On or about January 31, 2013 and times prior thereto, at the job site located at 8100 Cantrell Road in Little Rock, Arkansas, employees were using a step ladder positioned on the platform of a ladder jack scaffold to increase the working level height of employees. This condition subjected the employee to a fall potential in excess of eighteen feet.
